Chylous pleural effusion, a rare condition where lymphatic fluid accumulates in the chest cavity, has emerged as a concerning complication in patients with HIV-associated Kaposi sarcoma (KS). This condition, often linked to advanced infections or malignancies, poses diagnostic and therapeutic challenges due to its complexity and overlap with other pleural pathologies.
HIV and Kaposi Sarcoma: A Deadly Combination
Kaposi sarcoma, a cancer caused by human herpesvirus 8 (HHV-8), is strongly associated with HIV/AIDS. While antiretroviral therapy (ART) has reduced KS incidence, late-stage HIV patients remain vulnerable. The immune suppression from HIV allows HHV-8 to proliferate, leading to aggressive tumor growth and opportunistic infections.
Clinical Presentation and Diagnosis
Symptoms of chylous pleural effusion include chest pain, shortness of breath, and cough. Diagnosis typically involves imaging (CT scans) and thoracentesis to analyze fluid composition. The presence of chyle in pleural fluid—identified by high triglyceride levels—distinguishes this condition from other effusions.
Treatment Challenges
Management requires addressing both the underlying KS and the effusion. Surgical drainage, antibiotics for secondary infections, and ART optimization are common approaches. However, recurrence rates remain high, necessitating multidisciplinary care involving oncologists, pulmonologists, and infectious disease specialists.

This complication underscores the need for vigilant monitoring in HIV-KS patients. Early detection of pleural effusions can prevent respiratory failure. Additionally, research into targeted therapies for HHV-8 and chyle leakage mechanisms may improve outcomes.

Ongoing studies aim to clarify the pathophysiology of chylous effusions in KS patients. Innovations in minimally invasive procedures and antiviral treatments could revolutionize care. Patient education about symptom recognition is also critical for timely intervention.
